1928 Dated, Alcohol Prescription Form from the NATIONAL PROHIBITION ACT, filled out and completed.A very scarce Federal Treasury Department, Internal Revenue prescription form for a carefully controlled substance during the 1920s and 1930s - ALCOHOL! Note the use of Opium Poppies as the federal government's choice of decoration of the top border of a form regulating another type of drug! An interesting relic from a unique era in our history. This is a prescription for ''Spirits Frumenti'', whiskey! This document is printed in black and red was issued under the authority of the NATIONAL PROHIBITION ACT and was Blank No. 79 in Joseph V Maucher, MD's prescription book. This prescription was written on March 25, 1928, for U Murfey and cancelled on March 25, 1928, in Carrolltown, PA.
