This is a 45 rpm 7 inch single, which includes; My Name Is Jack (J. [John] Simon) 1-42539 Callee Music (ASCAP) 2:50, from The Cerberus Films Inc, Production You Are What You Eat on the A side; and There Is A Man (T. [Tom] McGuiness) 1-41949 Dick James Music Inc. (BMI) 3:58 on the B side. This is the 1968 US release on the Mercury Records label, with the catalog number 72822. Label colors are yellow-tan, and orange, with black lettering.

CONDITION: USED. A small hole has been drilled through both the sleeve and the record at the label. Otherwise, the picture sleeve is intact, even though it is soiled and creased. The vinyl has a couple of visible light scratches, that do NOT affect playing sound. It is also warped. Except for the drilled hole, the label is almost near mint. Overall this is a VERY GOOD record, in a GOOD sleeve.
