VIntage Cobalt Blue Square Glass Bottle . Once had a cork for closure. No chips and no cracks. There are striation lines on the neck and mold marks that stick out on each side of the neck, I have tried to show one side in last picture.

Height 5 1/4 inches 2 1/2 inches X 2 3/8 inches.
